Analysis of Needs for Sexual Education in Primary School Children.

The purpose of this study was to analyze and identify the higher-grade primary school children's needs for sexual education. We got the data with open question from 481 children from March to July 1999. We analyzed them according to the framework suggested by the Sex Information and Education Council of USA. The framework involves the six domains such as human development, relationship, personal skills, sexual behavior, sexual health, and society & culture. In the analysis of need for sexual education according to domain, need for human development was highest (79.2 %), after that followed society & culture (8.3 %), sexual health (5.8%), sexual behavior (3.6%), and relationships (3.1%). There was no need for personal skills. By topics, there were needs for puberty (38.0%), reproduction (32.5%), gender roles (6.0%), body image (4.9%), sexual abuse (2.4%) and reproductive health (2.4%). Girls and boys all mostly wanted to know human development 41.0 % and 38.2 % each. Society & culture (3.4 %), sexual behaviour (2.7%), sexual health (2.6%), relationship (2.4%) were domains selected by boys. Girls wanted to know society & culture (4.9%) and 'sexual health (3.2%). Comparing by grade, fourth, fifth and sixth grade children are commonly interested in development, society & culture. Fourth grade children scarcely show attention on the relationship, sexual behaviour and sexual health; meanwhile sixth grade children would like to know sexual behaviour and sexual health. When their grade became higher, they would like to know more precise, concrete and deep information. We suggest that the textbook/guidebook for sexual health for the higher-grade primary school children should be developed considering the sex and grade.

The impact of childhood experience has lifelong significance on subsequent health and development. Especially, the experience of infant is mostly affected by the quality of parental care and rearing environment. But the new mothers usually do not know what to do because of the lack of experience in these days. Therefore, an educational program regarding maternal role would be necessary. This study was conducted to evaluate the effectiveness of the maternal role education program for mother-infant interaction, child- rearing environment, and infant development. Non-equivalent control group time-series design was used, and Barnard's mother-infant interaction model was used as a conceptual framework of this study. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37 weeks, and their mothers. The final sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to September 3rd in 1999. For the intervention group, programmed education which focused on mother-infant interaction, breast feeding, and infant care was provided before discharge. Telephone counselling was provided within one week after discharge. Home visiting for maternal role education was provided twice, one month and three months postpartum. For the control group, home visiting was also conducted but only for data collection. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program was determined with repeated measure ANCOVA and t-test. The results were as follows: 1. Significant differences were found in mother- infant interaction between two groups(p= .000). It indicates that intervention program was effective in improving mother- infant interaction. In subscale analysis, four out of six subscale showed significant differences between the groups: sensitivity to cues (p=.000), social-emotional growth fostering (p=.000), cognitive growth fostering(p=.000) in mothers, and responsiveness to caregiver (p=.019) in infants. 2.The difference in the mean score of childrearing environment (HOME) between the intervention group and control group was significant(p=.003). When each subscale of HOME was examined individually, intervention group showed significantly higher scores in the diversity of stimulation(p=.000), and mother's involvement(p=.001). 3.Three-month-Infants of the intervention group showed higher GQ in the Griffiths mental development scale(p=.026). In subscale analysis, significant differences were found in the personal-social(p=.005), and the hearing and speech(p=.003). In conclusion, the maternal role education program proved to be effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results are very meaningful that we found maternal role education necessary for normal infants' mothers, and that nurses can make a great contribution in promoting health of infants and mothers.

Pain Relieving Effect of Intraoral Sucrose Replacement in Neonates.

The ability of neonates to perceive and react to pain, has been acknowledged recently. Recent researches have been shown that even short term pain can have lasting negative effects. We know that most of the anatomical pathway and neurotransmitter function necessary for pain perception are fully or nearly fully developed in the neonatal period. Many people are still reluctant to believe that pain felt by neonates may be as severe as that felt by older children or adults yet. The objective of the study is to assess and compare the analgesic effects of orally administered sucrose and pacifiers. And to determine the synergistic analgesic effects of sucrose and pacifiers. The tools for this study is Lawrence's NIPS (neonatal infant pain scale) with behavioural pain responses and index for pain physiological reponses as heart rate, respiratory rate and degree of SaO2. The participants are 96 healthy infants of neonatal age 1-7days and randomised to receive heel prick before 2minutes to blood sampling and physiological test in Nursery of K Medical University Hospital on May-July, 2000. The experimental group assigned to one of three treatment groups : no treatment; a pacifier; placebo(2ml 25% sucrose). Collected data were analyzed with the SAS program using X2-test, ANOVA and Duncan's multiple range test as post hoc. The results were as follows. 1) Pain behaviour responses: The pain score of placebo group(2ml 25% sucrose) is lower than no treatment group and pacifier group significantly(P=.000). Placebo group is different from no treatment and pacifier group with Duncan's multiple range test. 2) Pain physiologic responses (1) heart rate: The heart rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group significantly(P=.000). The heart rate change of placebo group is less than the other groups. (2) respiratory rate: The respiratory rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.2340). But, the respiratory rate of placebo group is lower than the other groups. (3) SaO2: The SaO2 score of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.3265). But, the change of SaO2 score of placebo group is less than the other groups. In conclusion, the sucrose placebo showed pain relief effect in behavior responses and less physiological responses. Accordingly, the sucrose placebo should be applied nursing intervention for simple pain management as heel prick.

The Hope, Burden, and Family Function in Mothers of Children with Cancer.

The purpose of the descriptive correlation study was to examine relations among the hope, the burden and the family function in mothers caring for children with cancer. 145 mothers completed the three questionaires of the study divided into tree sections: a) The Hope Scale, b) The Burden Scale, c) FACES-lll. The collected data was analysed with t-test, ANOVA, Scheffe test and Pearson correlation coefficients. The results were as follows: 1) The average age of mothers of children with cancer was 35.6 years old and the ages between 30 to 39 were the most abundant. 57.3% of the mothers had an education level of below high school education and 66.0% had religion. The average age of the children was 8.6 years old. Ages between 1 to 7 were the most and 60.8% were diagnosed as leukemia. 2) Regarding the section of degree of burden, mothers of children with cancer marked 2.6 out of 5, and the degree of hope 3.2 out of 4. The result for family function came out to be 3.5 out of 5, an average of family cohesion of 3.9 and family adaptation of 3.1. 3) There were significantly less burden to the mothers who were living together with a spouse compared to the mothers who were not. Also mothers who replied that they preserved good health came out to be exposed to less burden compared to the mothers who did not. In analysing hope according to the general characteristics of mothers of children with cancer, mothers who were employed marked high in the degree of hope compared to unemployed mothers. Furthermore, the degree of family cohesion marked higher with mothers who had higher education of college graduate, mothers with religion and mothers with a monthly family income of over W3,000,000, compared to the group of mothers with lower education of high school graduate, non-religious or with a monthly family income of less than W1,000,000. 4) Excluding the fact that the group sorted with children diagnosed as leukemia marked a perceivably high score regarding family cohesion, compared to groups with other cancers, the degree of burden, hope and family cohesion did not show any noticeable difference according to characteristics of children with different cancers. 5) In the correlation of the hope, the burden and the family function regarding the mothers of children with cancer, the burden did not have any manifest relationship with hope or family function. However, the degree of hope and family function cohesion had a direct proportional relationship, as family cohesion marked higher when the degree of hope were high.

A Sexual Knowledge and Attitude on the Exposure to PC Pornography of the Middle School Boys in Busan.

The purpose of the study is to identify the differences about knowledge and attitude between the subjects exposed to pc pornography and the control group not exposed to pc pornography. The study was executed from July 12 through July 16. The subjects were 423 middle school boys. The comparison points were their ethical sexual knowledges and sexual attitudes. The collected data was analysed by SPSS WIN(including multiplicand, percentage, x2-test, revised Fisher and t-test). The results of their study were as follows ; 1. The pc-related characteristics of the subjects. The subjects exposed pc pornography showed 51.8% rate of exposure to printed pornography. The control group showed the rate of 26.4%. The former group owned their pc(81.5%), however, the latter group showed 66.7%. The former group set their pc sets in their study rooms(66.1%) however, the latter group installed those in their study rooms(73.6%). The former group experienced internet pc communications(62.5%),however, the latter group showed 40.2%. The former group made use of their pc sets in recreation(entertainment) programs(77.0%), however, the latter group showed the rate of 67.8%. The former group showed that their school performance levelled up after their pc manipulation(80.0%), however, the latter group showed the rate of 86.2%. The former group replied that their pc manipulation contributed to their friendship(50.3%), however, the latter group showed the higher rate of 74.7%. The both groups replied that their family dialogs and contacts rather reduced (78.9% and 78.2% respectively). The both groups revealed that they reduced the TV watching the rate 76.5% and 48.3% respectively. The differences between two groups were surveyed in the level of 95% significance and the items such as "contacts to printed pornography, pc possession or none, pc use rate, friendship occasions and the reduced time of TV watching" showed the significant differences. 2. The differences of two groups' sexual knowledges. As to the sexual knowledges, the both groups showed 41.1 and 34.1 points against 100 points respectively. The statistical differences were significant(t=2.72, p=.007). The 5 items among 17 showed the significant differences between two groups. "The concept of masturbation" was x2=5.033, p=.025. "The prejudice to masturbation" showed x2=9.902, p=.002 "The difference between female and male as to sexual excitement" showed x2=7.985, p=.005. "The positiveness of masturbation" showed x2=10.205, p=.001. "The differences between two sexes as to sexual impulse and sexual desire" showed x2 =8.463, p=.004. In conclusion, The former group showed the higher knowledges than the latter group. 3. The differences of two groups' sexual attitudes. The 4 items such as "the curiosity to the other sex"(t=2.22, p=.027), "the attention to pornography"(t=3.39, p=.001), "the permission to pre-marriage intercourse"(t=2.15, p=.032) and "the preference to the female body exposure"(t=2.67, p=.008) showed the differences between two groups as to sexual attitudes.

The Effect of Self-esteem, Academic Achievement and Family Functioning of Adolescents on Hope.

This study is the descriptive survey for the purpose of providing the basic data that establishes the strategy to promote adolescent's hope by the examining of self-esteem, academic achievement, family functioning and hope of adolescents and the investigating of the factors influencing the hope in adolescents. The subjects for this study were 456 students of the first and second year of man's senior high school that located in Seoul. The data were gathered from 16th to 31st of the October 2000. For the survey tool, it was used that the Family Assessment Device(FAD) of Epstein, Baldwin & Bishop(1983), the Self-Esteem Inventory(SEI) of Coopersmith(1975), the class record order and Hinds & Gatusso(1991)'s Hopefulness Scale for Adolescents(HSA). The collected data was analyzed by statistics methods as the descriptive and frequency analysis, t-test, ANOVA, Pearson and Spearman correlation coefficients and Stepwise multiple regression of SAS program. The results of this study were following : 1. The mean score of self-esteem of young people was 51.06 +/- 6.83 and the mean grade was 2.04. The high academic achievement was 29.2%, middle grade was 52.7%, and low grade was 18.1%. The mean score of the family functioning was 38.30 +/- 6.98 and the mean grade was 2.25. The mean score of hope was 84.26 +/- 16.45 and the mean grade was 3.51. 2. The hope in adolescents was significantly different according to their father's school career. The mean score of the group that the father's school career was below junior high school was 77.32. That was significantly lower than the mean score 84.59 of the group that the father's school career was above college and the mean score 85.18 of senior high school group(F=4.04, P= 0.0183). 3. The self-esteem was represented the positive correlation with family functioning(r=0.43) and the all of the 4 subscales(r=0.31, 0.41, 0.39, 0.30). And, it was highly ranked as much as family functioning was good. The academic achievement was represented the positive correlation with self-esteem(r= 0.15). Also, the positive correlation was shown between the affective responsiveness, role recognition and emotional support as the subscales of family functioning and academic achievement(r=0.11, 0.12). And so, academic achievement was high as much as self- esteem was high and affective responsiveness and role recognition and emotional support were good. The hope was represented positive correlation with self-esteem and academic achievement(r=0.42, 0.26), and with the whole of family functioning(r=0.15) and the 4 subscales(r=0.13, 0.16, 0.11, 0.13). So, hope was high as much as self-esteem was high, academic achievement was high and family functioning was good. 4. The influencing factors on the hope of adolescents were self-esteem(17.63%), academic achievement(3.41%), father's school career(0.84%). These factors made it possible to explain 21.88% of hope.

Nursing Knowledge/Power and Practice in Pediatric Intensive Care Unit.

The purpose of this study was to reveal what influences the divergent methodological researches have brought the nursing practice in during the past 3 decades. The nursing record sheets ie, the nursing discourses were analyzed to know the knowledges that were recorded, accepted and communicated in nursing practice at pediatric intensive care units, and unclosed the philosophical and methodological position of that knowledges. The texts were 13 sheets, 3 kinds of nursing record(7 24hours flow charts, 4 nursing information record sheets and 2 transfer record sheets) used at 4 hospitals. The unit of analysis was 'word'. First, all words of the sheets were listed up, clustered into categories based on their contents. And then, the larger conceptual themes were drawn to elucidate the effect of the knowledge/power and the philosophical and methodological position of that knowledges. To enhanced the validity of the analysis, the data were analyzed by two researchers. The 'words' were classified into 3 categories; 'general information', 'assessment' and 'intervention'. The conceptual themes of the texts were 'the gaze for quantification and objectification' and 'technical/assimilated caring'. This themes reflected the logic positivistic and biomedical view that had dominated at clinical practice. Nursing has endeavored to resist the logic- positivistic knowledge/power and to established the nursing knowledge/power based on multiple philosophies and methodologies, especially phenomenological-interpretative. But the results of this study revealed that such efforts in nursing theory and research couldn't influenced the knowledge of practice. Logic positivism was yet so strong and the biomedical model yet dominated in the clinical practice. It identified that the borrowed theory and the knowledge from the received view gave nursing the power. But they were modalities that reinforced the dominant, medical power. Nursing has investigate the other positions (feminism, Habermas' critical social theory and Foucault's discourse theory). This positions suggest different assumptions but share the common concepts; equality, emancipation and freedom. The important point is how make these concepts the practical for nursing knowledge/ power in practice. We must recognize that the praxis at clinical setting take place at the field unlike theoretical praxis. The change of clinical practice is the social, economic and political change.

The Study on the Development and Effectiveness of Parent Role Education Program I: Focusing on the six-months results.

Recent research indicates that the new mothers want to learn about childrearing, and have burden in care of infants. Also it indicates that the new mothers say the lack of confidence and knowledge about infant care. This study was a prospective longitudinal reseach developing parent role education program and evaluating the effectiveness of this program for mother- infant interaction, childrearing environment and infant development. Longitudinal quasi-experimental design was used. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37weeks, and their mothers. The sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to December 14th in 1999. For the intervention group received programmed education that was consisted of discharge education, telephone counselling, and home visiting for maternal education. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program. This study was focused on the results of six months time point. The results were as follows: 1. There was significant difference in the mean score of mother-infant interaction(NCAST) between two groups. Intervention group showed higher scores in the subscales of sensitivity to cues, cognitive growth fostering, and caregiver total. 2. There was no significant difference in the mean score of child rearing environment (HOME) between two groups of six-month- infants. But when each subscale of HOME was examined, intervention group showed higher scores in the dimensions of opportunities for variety in daily stimulation, maternal involvement with child, and emotional, verbal response. 3. Six-month-infants of the intervention group showed significantly higher GQ in the Griffiths mental development scale. In conclusion, the maternal education program was effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results were very meaningful that we found parent role education necessary for normal infants' mothers, and nurses can make a great contribution in promoting health of infants and mothers.

This research objected to the diagnosed patients as acute lymphoblastic leukemia, acute myelogenous leukemia, neuroblastoma, non- Hodgkins lymphoma, Hodgkin's disease, kidney tumor, myelodysplastic syndrom and juvenile chronic leukemia after admission in the "P" hospital in Pusan from Aug. 1. 1999 to Jan. 31. 2000. The results of this study are summarized as follows. 1. On the specific character between the experimental(exp.) group and the control (con.) group : there were 7 of 4-7 years old patients(the most) in the experimental group(53.8%), 5 of 12 years old or older patients in the control group (38.5%). Patients who experienced operation were 7 in the exp. group(53.8%) and 6 in con. group(46.2%). The largest number of the patients' diagnosis was acute lymphoblastic leukemia by 5 in the exp. group(38.5%) and 4 in the con. group (30.8%). The hardest nausea came on the second day by 5 in the exp. group(38.5%), 9 in the con. group(69.2%). 2. P-score of the nausea vomiting on the number of daily anticancer drug administration : first day, the exp. group got 9.6 and the con. group 17.6(P = 0.03). 2nd day, 10.9 and 19.4(P = 0.00), 3rd day, 10.6 and 18.3(P = 0.00), 4th day 10.0 and 18.0, 5th day 10.9 and 16.8(P = 0.05). The score showed statistically significant difference(P < .05). 3.Oral intake didn't show statistically significant difference between two groups. However the average of Oral intake of the exp. group was continually higher than the con. group except to the first day after administration. In conclusion, nursing intervention and nutrition care are much more needed on the 2-3th day after administration to reduce nausea vomiting, and for remission of nausea and enlarging oral intake it is utilizable to apply the easy, economic Oral Cryotherapy to the young patients who undergo chemotherapy.

Development and Effect of Safety Education Program in Preschooler.

The purpose of this study was to measure the effect of safety education program in preschool children for accident prevention and improve their health through more systematic method. Data were collected from 584 preschoolers(247 preschooler are assigned to experimental group and 337 preschoolers are assinged to control group) from 4 to 6 years old using APP paper test which consists of questions and drawings. To experimental group, safety education were done 4 times within the time of 30 minutes per 1 time using education books, drawings, OHP. The findings of this study are as follows: 1.There were significant difference in movement( X2=18.732, p=.0000), behavioral character( X2=27.785, p=.000), synthetic judgement( X2=12.02, p=0.002). So, safety education program have effect on preschooler. 2.In the accident proneness on preschooler between experimental group and control group according to general characteristics, it proved significant difference in the case of accident prevention education were done, reasoning power( X2=10.48, p=.005), movement speed( X2=7.341, p=.025) and behavioral character( X2=18.86, p=.000), in the case of housing pattern is private house(individual house, yard?), reasoning power( X2=6.683, p=.035), movement speed( X2=12.76, p= .002) and behavioral character( X2=12.24, p=.002), in the case of housing pattern is mixed-type, movement speed( X2=6.935, p= .031) and behavioral character( X2=10.816, p=.004), in the case of housing pattern is over six stories, movement speed( X2=7.543, p=.023), in the case of subjects' age is 4 years old, movement speed( X2=16.5, p= .000) and behavioral character( X2=12.18, p=.002), in the case of subjects' age is 5 years old, movement speed( X2=7.519, p= .023), watchfulness( X2=6.372, p=.041), behavioral character( X2=14.74, p=0.001) and synthetic judgement( X2=14.5, p=.001), in the case of subjects' sex is male, life safety( X2=6.406, p=.041), movment speed( X2=22.86, p= .000), behavioral character( X2=13.72, p= .001) and synthetic judgement( X2=13.82, p=.001), in the case of subjects' sex is female, reasoning power( X2=12.57, p=.002) and behavioral character( X2=13.16, p= .001), in the case of childrens have past accidental experience, traffic safety( X2= 6.683, p=.035), in the case of childrens have no past accidental experience, reasoning power( X2=8.384, p=.015), movement speed( X2=20.6, p=.000), behavioral character( X2 =25.1, p=.000) and synthetic judgement( X2 =10.79, p=.005), in the case of children's order is first, reasoning power( X2=11.15, p=.004), movement speed( X2=11.92, p= .003) and behavioral character( X2=7.003, p=.030), in the case of children's order is second, movement speed( X2=6.694, p= .035), behavioral character( X2=26.9, p= .000) and synthetic judgement( X2=14.3, p= .001), in the case of nuclear family, reasoning power( X2=8.777, p=.012), movement speed( X2=19.0, p=.000), behavioral character ( X2=26.4, p=0.000) and synthetic judgement ( X2=9.999, p=.007), in the case of mothers' school career is under high school graduate, life safety( X2=8.023, p=.018), movement speed( X2=10.99, p=.004) and behavioral character( X2=6.777, p=.034), in the case of mothers' school career is beyond college graduate, reasoning power( X2=6.717, p= .035), movement speed( X2=8.963, p=.011), behavioral character( X2=25.03, p=.000) and synthetic judgement( X2=15.19, p=.001), in the case of mothers' age ranged 31-34, movement speed( X2=12.29, p=.002) and behavioral character( X2=14.17, p=.001), in the case of mothers' age ranged 35-39, movement speed( X2=9.859, p=.007), behavioral character( X2=9.095, p=.011) and synthetic judgement( X2=7.810, p=.020), in the case of mothers' age is over 40, life safety( X2 =5.593, p=.025), in the case of mothers' job is full-time, traffic safety( X2=6.032, p=.049) and reasoning power( X2=8.502, p= .014), in the case of mothers' job is part- time., movement speed( X2=10.99, p=.004) and behavioral character( X2=7.895, p= .019), in the case of mothers have no job, movement speed( X2=6.410, p=.041), movement stability( X2=6.879, p=.032), behavioral character( X2=27.72, p=.000) and synthetic judgement( X2=18.11, p=.000). The difference of accident proneness between experimental group and control group according to general characterists, it also showed that there were significant difference in behaviorl character compared to other area.. From this findings, we can guess that safety education program change and guide preschoolers' behavioral character to desirable direction.
