PURPOSE The aim of this study was to analyze the correlation of gestational age and birth weight with weight gain of very low birth weight infants(VLBWI) during their hospital stay. METHOD This is a 5 year retrospective study of which data were collected through review of medical records. Subjects were 124 VLBW infants with a birth weight more than 1000g and less than 1500g who received neonatal intensive care at the university hospital between January 1, 1997 to December 31, 2001. RESULT After calculating the z scores of birth weights and discharge weights, z scores of discharge weight and birth weight were compared with the median weight of a fetus of comparable gestational age based on an intrauterine growth reference. There was a significant difference between z scores of birth weight and discharge weight(t=11.60, df=122, p=0.000). Regardless of intensive care during the prolonged hospital stay, VLBW infants showed slow growth rate compared with the median weight of a fetus of comparable gestational age. CONCLUSION VLBW infants developed a poor velocity of weight gain during the prolonged hospital stay after birth. The development worsened during the period of physiological weight loss and regain, and they did not reach to comparable growth rate of normal fetus even at the time of discharge.
This poor growth velocity of VLBW infants influence negatively for their future growth. Therefore nureses who work at the neonatal intensive care unit must develop an effective nursing intervention protocol to promote the velocity of weight gain and to conduct the parental educational sessions to emphasize the importance of weight gain for VLBW infants at home.

PURPOSE The purpose of this study was to identify the effect of hand acupuncture on reduction of headache. METHOD This study used a quasi-experimental pre-test and post-test design. 45 middle school girls who complained of headache were selected as subjects. The study lasted from November 2002 to June 2003. There were 22 girls in the experimental group while 23 girls were control group; convenience assignment was used. The two groups were homogeneous on characteristics. The experimental group received hand acupuncture. The treatment was to puncture corresponding points (A33, B27, M1, I2) on both the palm and the back of a hand with disposable acupuncture needles, and then remove them after a 20-30 minutes recess during which the subjects reclined on a bed. The data were analyzed using the SPSS PC with Fisher's Exact X(2)-test, and Mann Whitney U-test. RESULT The results of this study were as follows: 1) In the experimental group, objective pain decreased after hand acupuncture (U= -2.51, p= .01). As for the objective pain degree, a significant reduction in voice change was observed in the experimental groups (U= -2.35, p= .02). Facial expressional changes were observed (U= -1.87, p= .06) and perspiration degree (U= -.99, p= .32) has reduced. However, these differences were not statistically significant between the experimental and control group. 2) Subjective pain degree decreased after hand acupuncture in the experimental as compared to the control group. 3) No statistical significant difference in comfort (U= -.29, p= .78) or distress (U= -1.51, p= .13) between experimental and control group were found. CONCLUSION These findings indicate that hand acupuncture is an effective method for reducing headache. Therefore, hand acupuncture can be considered as an independent nursing intervention for headache reduction.

PURPOSE This study aimed to identify the alcohol drinking pattern of parents and to analyze drinking related factors in problem drinking among adolescent's parents. METHOD This study was conducted through a structured questioning from November 5th to 22th in 2002. Those are 2,522 parents of the students of the 1st and 2nd grade who were selected randomly among 26 middle and high schools located in Socho-gu, Seoul. Data was analyzed using SAS program that included descriptive statistics, Chi-square test, and multiple logistic regression analysis. RESULT 1. For frequency of drinking, 33.1% of the subjects responded to drink two-four times a month, 26.0% drink more than two times a week. 2. For the amount of drinking, 29.6% drink one or two glasses while 9.4% drink over ten glasses at a time. 3. The mean AUDIT score was 8.27+/-7.02(score of range: 0-40). 4. Based on AUDIT score, drinkers with less than eight point from AUDIT(normal group) were 55.1%, from more than eight point to less than twelve point from AUDIT(problematic drinker) were 16.3%, and more than twelve point from AUDIT(alcohol abuse and dependency) were 28.6%.
5. Based on more than twelve point from AUDIT, female(mother), high school and, have religion, housekeeper were the significantly higher score than counter parts. CONCLUSION Alcohol drinking pattern and level of drinking of their parents links to their children's drinking and results in family, social, and national loss. Active prevention is needed. Specially, pertinent education about drinking and public education for mothers who are housekeepers should be carried out with concentrated intervention programs for the problematic drinker so alcohol abuse and dependency can be reduced.

PURPOSE The purpose of this study were to develop a respiratory communicable disease prevention program for preschoolers and measure it's effects. METHOD The respiratory communicable disease prevention program for preschoolers consisted of texts, cartoons, photographs, discussions, demonstrations, puzzle games, die games, compensation/reinforcement, and token economy which were directed under the systematic design of instruction by Dick & Carey. This study was a quasi experimental study under the nonequivalent control group with pretest-posttest design. The subjects of this study were 45 preschool children who are attending 3 different district nursery schools and they were matched by the age, pretest knowledge, and pretest behavior. The instrument used in this study was criterion referenced test items that were developed by a researcher for evaluating the subject's knowledge, attitude, and behavior about respiratory communicable disease prevention. A pretest was administered a week before treatment. Experimental group I was administered by the treatment of respiratory communicable disease prevention program. Experimental group II was administered by above program with token economy program. The posttest was conducted on the eighth day. The third test for behavior was completed 15th day. To determine the effect of the program, the data were analyzed by the SAS 6.12 program with Kruskal Wallis test, ANCOVA, ANOVA, Duncan's test and paired t-test. RESULT 1) There was a significant difference in knowledge between the experimental groups and control group(F=5.89, P=0.0197). 2) There was a significant difference in attitude between the experimental groups and control group(F=3.29, P=0.0469). 3) There was a non-significant difference in behavior between the experimental groups and control group(F=0.00, P=0.9512). 4) In the experimental group II, there was highly significant increase in behavior after token economy(t=4.5252, P=0.0005). CONCLUSION It was found that the respiratory communicable disease prevention program for preschool children was effective in changing the preschoolers' knowledge and attitude on the respiratory communicable disease prevention, but not enough for changing the preschoolers' behavior.
Token economy was improved as an effective and strong method for inducing desirable changes of preschoolers' behavior.
PURPOSE The purpose of this study was to explore the fatigue of the mothers of the hospitalized children's mothers in descriptive study. METHOD Two hundred eighty three mothers who take care of the hospitalized children in three University hospitals were enrolled from June, 1 to October, 30, 2003. Data were collected using a questionnaire titled "Symptom Table on Fatigue Perception" designed by the Fatigue Research Committee of Japan, consisted of a total 30 items categorized into three sub-dimensions: the physical, the psychological and the neuro-sensory. SPSS was used for the analysis of data with 0.05 of significance. RESULT 1) The mean average fatigue score of subjects was 1.94(SD:.48): the physical fatigue revealed the highest value with a mean of 2.19(SD:.57), followed by psychological fatigue with a mean score of 1.85(SD:.52), neuro-sensory fatigue with a mean of 1.79(SD:.51). 2) With the respect to the general characteristics of hospitalized children and mothers, there were statistically significant differences in the mothers' fatigue perception by the child sex(t=-2.697, p=.008), the character of child(F=9.032, p=.000), the child condition compared to pre-hospitalization(F=3.523, p=.031), with or without support in domestic households(t=-1.981, p=.049), the amount of sleeping time(t=2.704, p=.007), and with or without of night-time sleep disturbance because of the child hospitalization(t=2.082, p=.038). CONCLUSION These results suggest that health care professional need to be aware of the persistent presence of the maternal fatigue related to the child hospitalization and the factors worsen the degree of the fatigue.
PURPOSE Adolescence is viewed as a critical period in the formation of health behavior and many health behaviors developed during this era persist into adulthood.
Social-cognitive theory, self-efficacy as a central construct, has been used to predict and intervene the health behavioral patterns in adolescent. Previous research demonstrated that the attachment in childhood and self-esteem as psychosocial factors are predictors of the health efficacy in adolescent and it is viewed as solely an antecedent for the lifelong health behavior. METHOD To investigate the path pattern of attachment in childhood, self-esteem and health efficacy in adolescents, 381 adolescents in high school from two urban cities in Korea were recruited for cross-sectional sample. Attachment in childhood was measured using Mother-Father-Peer Scale.
Self esteem was measured with Hare self esteem. Perceived self efficacy in Health was measured by the School Health Efficacy Questionnaire. The path analysis revealed a significant relationship between attachment in childhood and self-esteem, self-esteem and health efficacy in adolescents.
Self esteem was the strongest contributing factor for health efficacy in adolescent. The results suggest that attachment in childhood may aid in formulating positive self -esteem in adolescents and self-esteem played a major role in predicting health efficacy in adolescents. CONCLUSION Therefore, self-esteem enrichment should be incorporated with adolescent health promotion and certainly should be a component in any health education program in school health program and interventions. These results have implications of psychosocial and family related factors on health promotion and health education for the health care provider with regard to primary and secondary prevention in adolescent population.
PURPOSE This study was to confirm the effect of acupressure on the emesis control and the weight change among pediatric cancer patients receiving anti-cancer chemotherapy. METHOD Forty pediatric cancer patients, receiving the induction stage of chemotherapy with MTX and vincristine, were divided into control(n=20) and the intervention group(n=20). Both groups received regular anti-emesis medication, but the intervention group was added acupressure maneuver for 5 minutes on P6 point for 3 times a day for 5days: before chemotherapy, lunch and dinner by investigator during the hospitalization and by mother at home. The instruments for this study were Rhode's(1986) Index of nausea, vomiting and retching(INVR), Cas electric scale and pamphlet developed by researcher. RESULT Significant differences in the degree of nausea and vomiting were observed between the control and the intervention group as measured by INVR(t=4.73; p=.01).
Repeated measures ANOVA also shows that the group effect was significant(F=22.39, P=.01) as was the time effect(F=380.35, P=.01). The group by time interaction was also significant(F=5.27, P=.01). Acupressure maneuver was apparently effective in reducing the degree of chemotherapy-induced nausea and vomiting. There were also statistically significant weight loss noted in the control group than the intervention group(t=5.42, p=.01). CONCLUSION Acupressure on P6 point shows an effective adjunct maneuver in reducing the degree of nausea and vomiting and conserving the weight in pediatric cancer patients. Therefore, it is proposed that acupressure should be applied as supportive nursing intervention strategies to relieve chemotherapy induced nausea and vomiting and to prevent weight loss in pediatric cancer patients.
PURPOSE There has been a number of reports on elution of bisphenol-A, an endocrine disrupting chemical, from milk bottles, all concerning the potential health effect of the substance. In this study, we measured the elution of bisphenol-A from milk bottles during boiling-water sterilization, to suggest a safe sterilization method. METHOD Through the survey of 200 mothers, 78.5% of them expressed their concern on the possible harmful effect of bisphenol-A, which might be eluted from the bottles. And it was found that most mothers use boiling-water to sterilize bottles; duration of boiling bottles could be divided into three groups of 3, 5, and 10 minutes. We measured the elution of bisphenol- A from three different brands of bottles, subject to various mode of sterilization found in the survey. RESULT In all the measurements, the concentration of the eluted bisphenol-A was in the range of 0.3 - 0.7 ppb, far below 2.5ppm, the level set up by Korea Food & Drug Administration(KFDA) for a potential harmful effect. CONCLUSION Although trace amount of bisphenol -A can be eluted from polycarbonate milk bottles during sterilization, and bottle feeding, the amount is found to be too small to cause any health related impact for infants.
PURPOSE The research was conducted to investigate the experience of maternal role attainment of mothers of premature infants admitted in NICU and to conceptualize the phenomena. METHOD The grounded theory method was utilized for data collection and analysis. 8 mothers of premature infants were selected and in-depth interview was performed. Paradigm model was utilized for data analysis and presentation. RESULT The central category was "unstable maternal identity". The properties of the core phenomena was "ambivalent feeling to baby" "negative emotion" "commitment to baby". The loss of control due to premature delivery was the causal condition. contextual condition was the "perceived threats" due to severity of the premature infant and uncerainty of the baby's life. The mother's health status, economic status, and familial and social support was recognized as intervening conditions during the process of maternal role attainment. The strategic action/interactions were emotion-focused coping, reappraisal of the situation, problem-focused coping, and information seeking. The consequence was the maternal role attainment with competence and expectation. CONCLUSION The process of maternal role attainment was affected by threats due to severity of the baby and intervening factors and interaction strategy. Further research is recommended to develop adequate intervention method during the process of maternal role attainment.
